Nathan R. Jaisingh, NPPresident & CEO
Nathan R. Jaisingh, is a board certified psychiatric nurse practitioner who has studied at Morehead State University, in Morehead, KY, Rush University in Chicago, IL, and the University of Kentucky in Lexington, KY. His graduate and doctoral areas of focus include adolescent, adult, and geriatric psychiatry, psychopharmacology, and clinical practice & research with competitive athletes in improving overall performance through mental strength coaching.
Nathan R. Jaisingh founded Benchmark Psychiatric Services, LTD. in 2002, with the primary endeavor centered on geriatric psychiatry and psychopharmacology - with the goal of delivering stellar services on-site to long term care facilities. This model has grown from a single provider in two facilities, to now 15 providers in nearly 65 facilities in Illinois and Kentucky - and still growing!
After two and a half years of research and development, Nathan R. Jaisingh identified a significant need for a comprehensive program specifically structured for professional, collegiate, and competitive pre-collegiate athletes. From this, Integrative Athletic Enhancement (IAE) was created.
Nathan R. Jaisingh is a Frank G. Dickey Fellow of the University of Kentucky and is an avid University of Kentucky athletics supporter. He is also a fervent Chicago Bears and Chicago White Sox fan, having now lived in the Chicagoland area since 2000.
Nathan R. Jaisingh is married to a wonderful wife, Nicole, and together raise 3 delightful & inspiring children.
Amy Pawlowski, LCSWLicensed Clinical Social Worker
Amy Pawlowski is a Licensed Clinical Social Worker who has been practicing for fifteen years. She earned both her Bachelor's degree and her Masters of Social Work degree from Indiana University Northwest. Additionally, Amy has completed four years of advanced psychotherapy training at the Institute for Clinical Social Work in Chicago.
Amy's experience has included working in community mental health, as well as in private practice. She worked eight years within the criminal justice system which included Indiana State Prison, Parole, and both Federal and County Probation departments. Her experience also includes working with severe mental illness, co-occurring disorders, and various addictions by utilizing concepts such as DBT, CBT, mindfulness and relational theory.
Amy enjoys reading, practicing yoga, spending time with family and playing with her animals; four dogs and three cats.
Andrew Segovia Kulik, MDMedical Director
Dr. Segovia Kulik completed his Bachelor's of Science in both Biology and Psychology at Birmingham-Southern College in Birmingham, Alabama. He proceeded and completed his Medical Degree at Grace University School of Medicine. Dr. Segovia Kulik then completed one year of Family Practice Residency at Jackson Park Hospital (JPH) on the south side of Chicago. During his time at JPH, Dr. Segovia Kulik decided to continue serving an underserved urban population. However, he chose to change to a different medical discipline, Psychiatry.
In January of 2001, Dr. Segovia Kulik began training in the University of Illinois at Chicago (UIC) Department of Psychiatry Residency Program. He completed 3.5 years at the University which ended in the summer of 2006. Dr. Segovia Kulik was honored at the start of his final year at UIC by being elected Chief Resident supervising 46 other M.D's in the department of Psychiatry. This title was an honor due to the democratic process adhered to by the University decided amongst his contemporary peers. Dr. Segovia Kulik graduated from UIC in the summer of 2006.
During Dr. Segovia Kulik's training in general psychiatry he became intrigued with the analytical and adversarial nature of Forensic Psychiatry. He applied and was accepted to one of the premier and most well established Forensic Psychiatric training programs in the country. He attended and trained at the University of Maryland in Baltimore, Maryland from the summer of 2006 until the summer of 2007.
During his forensic psychiatric training Dr. Segovia Kulik accepted a Forensic Psychiatric position at Forensic Clinical Service (FCS) of the Circuit Court of Cook County in Chicago, Illinois. This position is one of the foremost full time forensic psychiatric positions in the country regarding experience and breadth of exposure to criminal forensic issues.
Additionally, after working at FCS for almost two years, Dr. Segovia Kulik received a promotion within the Cook County system and he accepted the position of Divisional Chairman of Consult Liaison in the Department of Psychiatry at Stroger Hospital of Cook County (formally Cook County Hospital) in Chicago, Illinois. Lastly, while Dr. Segovia Kulik transitioned to his new full time county position, he also accepted and became Medical Director of Benchmark Psychiatric Services, LTD.
Carl Cavitt, RNPsychiatric RN Liaison
Mr. Cavitt graduated with a B.S. in Nursing from Olivet Nazarene University in 1972. His experiences have included working with the State of Illinois in the Division of Long Term Care, Mental Health and Public Health. During his tenure with the State, he inspected and consulted with nursing homes, assisted and supportive living facilities. In mental health, he supervised other nurses and mental health technicians and was also the employee’s health nurse. His State public health service included licensing hospitals. He was Director of Personal Services for the Fulton County Health Department. During this time he supervised 20 nurses and started up a Medicare certified home health agency, a community blood pressure and diabetic screening clinic and worked with the local county mental health agency.
Carl has volunteered many years with the Illinois-Michigan Lions Eye-bank. He also volunteered for the American Red Cross, Disaster Services. He has gone on numerous local and national disasters.
Charles Preyar, LCSWLicensed Clinical Social Worker - Psychotherapist
Charles Preyar received his A.B. from Lincoln University of Missouri and his M.S.W. from the University of Illinois of Chicago. He is a certified Licensed Clinical Social Worker and a member of The Academy of Certified Social Workers.
Charles provides services to aid his colleagues in career development, such as clinical supervision for assistant social workers and other counselors.
Clinically, he has a focus and expertise encompassing Depression and Anxiety disorders in the context of the individual, family, and group setting.
Charles has provided services in outpatient clinics, social service agencies, churches, private homes, and currently through Benchmark Psychiatric Services, LTD. in long term care facilities.
Chrysta PozdolDirector of Operations
After working for 10 years in non-profit, Chrysta Pozdol joined Benchmark in 2014. As the Director of Operations, Chrysta enjoys creating support and organizational systems for the Benchmark providers and staff, ensuring they are equipt to deliver the highest level of quality care to our patients. Directly assisting President and CEO, Nathan Jaisingh, Chrysta coordinates operational procedures, manages the administrative support team, and directs develpment to ensure the vision and mission of Benchmark Mind & Health is being engaged throughout all divisions of the organization. She works closely with the marketing and buisness growth department to expand brand recognition and relationships both online and in the community.
Chrysta has also worked professionally as a singer, songwriter, and musician. She currently volunteers as song leader at Smith Crossing Retirement Community Sunday service through Parkview Christian Church. She enjoys practicing yoga, teaching music lessons, and spending time with beloved family.
Donna Platt, LCPCLicensed Clinical Professional Counselor
Donna Platt is a licensed clinical professional counselor (LCPC). She received her Bachelors of Science Degree in Organizational Psychology and a Masters Degree in Counseling Psychology from St. Xavier University in Chicago. She is certified in Reality Therapy/Choice Theory and is a certified Addictions Counselor (CADC) in the state of Illinois. Donna completed specialized training in faith-based counseling and volunteers as a commissioned Stephen Minister at Parkview Christian Church in Orland Park, Illinois. She is also a Certified Positive Discipline Parent Educator, a theory that teaches important social and life skills - raising young people to be responsible, respectful and resourceful members of their community.
Donna has enjoyed counseling children, adolescents, adults and their families in a variety of settings. She served as a clinical therapist for inpatient populations at Linden Oaks Hospital in Naperville, Illinois. She also worked as a lifestyle coach for ProActive Kids, a program that addresses mental health issues for children with obesity.
Donna utilizes several theories in her treatment of her clients, and finds Acceptance and Commitment Therapy (ACT) to be especially helpful. This theory assists clients in achieving freedom from the limitations of depression and anxiety by teaching clients how to become the author of a more meaningful and value-driven life.
Joan Slawinski, LCPC, CADCLicensed Clinical Professional Counselor
Joan Slawinski, LCPC, CADC earned her Masters Degree in Counseling and a Minor in Addiction Studies from Governor's State University. She is a Licensed Clinical Professional Counselor and a Certified Alcohol and Drug Counselor and has experience working with adolescents and adults with mood and anxiety disorders, relationship and family issues, substance abuse problems, and anger and stress management difficulties.
Joan has worked in a variety of settings including hospital, residential, and outpatient. She realizes that coming into therapy takes a lot of thought and possibly some fear and discomfort. She uses a collaborative and compassionate approach to help clients in a warm, non-judgmental environment.

I believe that the most important factors in successful therapy are the trust, sharing and respect that you receive from the therapeutic connection between us. My goal is to help clients create more meaning, balance, and happiness in their lives.
As the Receptionist at Benchmark Mind + Health, Judy is the smiling face and pleasant voice that you will be greeted by when you enter or call the office. She is, most likely, the person who will check you in for your appointment and assist you with scheduling your follow up appointement. Judy has over 15 years' experience working in medical administration and provides excellent support on many levels including patient care quality, scheduling, communication, and billing.
Judy is a very proud mother and grandmother. She and her husband reside in New Lenox, IL. In addition to her love of couponing, she is an avid Sudoku player and loves horseback riding.
Kathryn Cavitt, NPCertified Nurse Practitioner
Kathryn Cavitt is a Board Certified Family Nurse Practitioner with a particular interest in psychiatric care and long term care. At Benchmark she is a full-time provider, seeing patients for psychiatric evaluations and management of medications. Additionally, Kathryn is available to see patients in the outpatient setting atBenchmark's office in Orland Park.
Kathryn has been a nurse since 1974, having graduated from St. John's School of Nursing in Springfield, Illinois. She then continued on with her education, earning a Bachelors of Science/Nursing Major at Olivet Nazarene University in Kankakee, Illinois in 1988, and a Masters in Science/Community Health at St. Xavier University in Chicago in 1997. She holds a Family Nurse Practitioner national board certification from the American Nurses Credentialing Center.
Ms. Cavitt has held N.P. positions in long-term care, family practice, pediatrics, internal medicine, and hospice and palliative care. She has also held adjunct and part-time nursing professor positions for Olivet Nazarene University, Kankakee Community College, and University of St. Francis in Joliet. Kathryn has also served as a preceptor for nurse practitioner students, providing experience for them in an outpatient internal medicine setting.

Laura Lorenz, LCSWLicensed Clinical Social Worker
Laura Lorenz is a certified Licensed Clinical Social Worker. She completed her Bachelors of Arts in Psychology from Trinity Christian College and her Master’s in Social Work from University of Illinois at Chicago.
Laura has experience in non-profit community mental health setting working primarily with children, adolescents, young adults, and their families through individual, group, and family counseling. Her clinical interests include helping individuals with a variety of mental health issues including depression, anxiety, school issues (academic and behavioral), and familial issues.
Laura has experience in both the clinical and school setting working with individuals on the Autism Spectrum. She enjoys working with individuals on the Autism Spectrum and their families in order to develop and enhance healthy Social Skills and address mental health needs. Laura utilizes a variety of empirically based treatment theories in order to meet each client’s individual needs. She enjoys incorporating expressive therapy techniques including the use of art and mindfulness in her individual and group work.
Laura and her husband, Mike, are blessed with two beautiful young girls who keep them laughing daily. As a family they enjoy spending time outdoors playing and fishing. They are also avid sports fans, especially hockey and football.
Lisa LundhOffice Manager
As Office Manager since 2009, Lisa is a central column of our office operations team. If you are a patient, you are likely to speak with Lisa regarding scheduling, discussion of billing matters, your insurance coverage or related. Lisa provides support on many levels including provider scheduling, human resources, patient care quality, and she works tirelessly to ensure the office is a clean, comortable, and safe environment for our providers and patients.
In addition to being the proud Mother of a former US Marine and a doting Grandma, Lisa is a cancer survivor. She recently fought and beat Lymphoma and has been in remission since 2012. Lisa resides in Orland Hills, IL and is an avid Chicago Blackhawks fan.
Mary Wells, NPBoard Certified Adult-Geriatric Nurse Practitioner
Mary B. Wells is a Board Certified Adult-Geriatric Nurse Practitioner with a long standing interest in psychiatric care and long term care. At Benchmark she sees patients for psychiatric evaluations and medication management. She is available to see patients in the outpatient setting at Benchmark’s office in Orland Park.
Ms. Wells graduated from the University of Oklahoma with a Bachelor Degree in Nursing in 1979. She obtained a Master of Nursing degree from Aurora University with a specialization in Nursing Informatics (computers) in 1988. In 2000, Ms. Wells obtained a massage therapy diploma. She obtained her Adult/Geriatric Nurse Practitioner Master’s degree in 2014 from Maryville University in St. Louis, MO. She holds Adult/Geriatric Nurse Practitioner Certification from the American Association of Nurse Practitioners.
For over 10 years, Ms. Wells held positions at two different state mental health facilities. She has worked in Long Term Care, taught in the nursing program at Aurora University and taught in the massage and certified nursing assistant programs at Waubonsee Community College. She was Dean of Student Services at a private massage therapy school. Ms. Wells owned and operated a private medical massage therapy business and employed 5 therapists and 15 continuing education providers for over seven years. She has also worked as an occupational health nurse and as a Nurse Practitioner in the State of Illinois prison system.
To de-stress, Ms. Wells works with fiber of all kinds – Knitting, spinning, weaving, basket-making, papermaking, sewing and print-making as well as many other fiber modes. She also enjoys yoga, massage, and baking.
Michelle Mannino-Purkey, MS, LMFTLicensed Marriage & Family Therapist
Michelle Mannino Purkey received her undergraduate degree in Psychology from Purdue University and her master's degree in Marriage and Family Therapy from Purdue University Calumet. Her clinical experience includes working with individuals, couples, families, and groups.
Michelle's areas of interest include medical family therapy and bereavement. Specifically, Michelle completed her master's thesis on the effects of cancer on the family. She compiled information about cancer support centers throughout the United States and the types of services they offer to people affected by cancer.
Michelle works from solution-focused approach and believes that people are the experts on their lives. She views her role as someone who can offer support and empathy as well as someone who can challenge and encourage people to find new solutions. Michelle believes that discussing and highlighting a person's strengths and prior successes is a critical part of therapy.
Sheryl Ankrom, LCPCLicensed Clinical Professional Counselor
Sheryl Ankrom is a Licensed Clinical Professional Counselor (LCPC) with over 15 years of experience in behavioral health services. Sheryl believes that it is imperative to understand each client's uniqueness and uses a collaborative therapeutic approach that is based on the client’s individual needs and perspectives. In a nurturing and supportive environment, Sheryl focuses on identifying and building upon each individual’s strengths and guides clients through the process of modifying problematic issues and attaining personal goals.
In private practice, community non-profit and hospital settings, Sheryl has worked with child/adolescent, adult and geriatric clients with a broad range of emotional, behavioral, and mental health issues. She has also received specialized training to work with survivors of domestic violence. Using individual, family and group treatment methods, Sheryl employs empirically supported techniques in her treatment strategies. She has extensive experience in assessment, diagnosis and treatment of a variety of psychological difficulties, including mood disorders, anxiety disorders, personality disorders, addiction issues, relationship problems, parenting issues, and unhealthy expressions of anger.
Sheryl received her undergraduate degree from St. Xavier University in Chicago, Illinois and received her Master's of Science in Counseling Psychology from Capella University in Minneapolis, Minnesota. She is licensed in the state of Illinois as a clinical professional counselor and also holds the designations of National Certified Counselor (NCC) and Certified Clinical Mental Health Counselor (CCMHC) through the National Board of Certified Counselors. As a former mental health writer for About.com, a New York Times Company, Sheryl has numerous published articles relating to the symptoms, causes, treatments, and self-help strategies for individuals with anxiety disorders and their families.
Tammy Vosler, RNLTC Clinical Coordinator & Nurse Liaison
Tammy Vosler graduated with a B.S. in Nursing from South Dakota State University after having grown up in the State of South Dakota. She has spent the vast majority of her 23 year nursing career in child, adolescent, and adult psychiatry at Rush Presbyterian St. Luke’s Medical Center and at Mt. Sinai, Hartgrove and Silver Cross Hospital’s. She has been a nurse manager, nurse educator, charge nurse and school nurse all in the Chicagoland area. She is excited to utilize her past employment experiences to help her in her role as Long Term Care Clinical Coordinator and Nurse Liaison at Benchmark.
As the clinical coordinator for Benchmark’s 22+ contracted facilities, Tammy has clinical interaction with our providers, the facilities we service, and Benchmark each day, to ensure we are providing excellent care to the residents as well as the facilities we serve.
Tammy, is also the Intake coordinator for Benchmark, so if you are a new patient, it’s her voice you may hear on the other end of the phone coordinating details for your first visit with a Practitioner or Counselor. She hopes this is a supportive, caring phone call that leaves you feeling more at peace and full of hope as you begin your journey with Benchmark.
Tammy has been married for 23 years to Keith. They have 6 children and 8 grandchildren, which keep them busy and happy. However, that is not the extent of their family. 32 years ago Keith started and is the director of a ministry in the Englewood area here in Chicago. The mission is to reach out as a light of hope and a place of refuge to primarily urban, at-risk, unchurched youth and their families. Many of these youth come to Keith and Tammy’s home to do laundry, get a hot meal, find some peace and safety and just hang out with the family. They bring much laughter and fun to our ever growing family.
Tim CullnanDirector of Business Development
Tim Cullnan has over 30 years’ experience is all areas of Marketing and Advertising. He earned his B.A. from Loyola and his M.A. from De Paul Universities. After a career with major agencies, he launched his own consulting company, DAS, Inc. Through it, Tim now specializes in Business Development for both existing and start-up companies.
He lives in Palos Heights, IL with his wife, Peggy. Together they have raised four children and are the proud Grandparents of 3 boys.
Tom McCann, MA, LPCClinical Liaison
Thomas McCann received his Masters of Arts in Gerontological Counseling from The Adler School of Professional Psychology.
Under the supervision of Laura Lorenz, LCSW, Thomas currently provides counseling services and clinical liaison support at Benchmark's contracted long term care facilities. His areas of interest include providing insight and relief through psychotherapy with adults and the elderly.
Thomas was born in Mundelein, Illinois. Tom enjoys sports such as track and field and basketball. Other interests include hiking and reading classic literature.
